Re-Energize
What is Re-Energize?
Re-Energize is an accredited educational support program developed to prevent or delay the onset of type 2 diabetes and other chronic diseases such as asthma, high blood pressure, arthritis, heart disease and some cancers. Re-Energize can also help to alleviate conditions that often run hand-in-hand with chronic disease, like depression and anxiety.
The guidelines that Re-Energize follows do not involve complicated calorie counting, meal replacements, or special dietary products. These are suitable for the whole family and support practical behavioural changes that are realistic for long-term benefits.
The program consists of seven sessions conducted over a 6 month period that will educate participants about pre-diabetes, goal setting, nutrition, smoking and alcohol consumption, physical activity, stress reduction techniques, and how to maintain these behaviours and changes for life.
At the end of the seventh session, the Re-Energize participant will:
- Understand the risks of type 2 diabetes and other chronic diseases and associated lifestyle factors
- Recognise the importance of regular diabetes screening
- Be aware of the benefits of health lifestyle changes such as healthy eating and an increase in physical activity
- Increase their capacity to formulate behaviours strategies that support he adoption and maintenance for lifestyle changes with an increased ability to self monitor behavioural changes and the self manage their own health
- Understand the benefits of reducing alcohol consumption and smoking cessation (if applicable)
- Have access to relevant community programs and resources enabling them to sustain lifestyle changes
How do I sign up for Re-Energize?
Referral to Re-Energize can happen in a few different ways.
- General Practitioner referrals can occur after completion of the associated Medicare Benefits Scheme (MBS) Items - the Type 2 Diabetes Risk Evaluation, the 45yo to 49yo Health Check, and the Aboriginal and Torres Strait Islander Health Check
- General Practitioners can also refer using a General Consultation, if the patient is not eligible through the Chronic Disease Item Numbers
